What is Advance Care Planning?

Advance care planning is a dynamic process that helps people make informed decisions about health care preferences based upon their values and beliefs. It encourages reflection and discussion with their health care providers and circle of support to determine health care goals, care and treatment.

As people age and their life circumstances and health conditions change, it is important to regularly review advance directives and communicate health care preferences. If they are frail, elderly, or have a serious illness, they may want to ask their health care provider about an IPOST (also available in Spanish).
