Your Care Team

The Iowa City Hospice care team begins with YOU, the patient. You continue to direct your own care, with the guidance and support of your personal physician. The following care team members are available, based on your individual needs and preferences:

  • Staff medical directors, who partner with your personal physician and other healthcare providers to ensure consistent, personalized care
  • Registered Nurse Case Managers, who are available 24/7 to answer your questions or address pain and symptoms – even in the middle of the night
  • Staff pharmacists, who consult with your care team to ensure the best possible pain and symptom management
  • Hospice aides specially certified in hospice care, who provide help with your personal care, such as bathing and getting dressed, while respecting your dignity.
  • Social workers, who address you and your family’s emotional needs and help you through confusing paperwork
  • Spiritual counselors, who respect and support you in your own spiritual or religious beliefs and practices
  • Bereavement specialists, who offer grief support services to your friends and family, and to anyone in the community who has lost a loved one, regardless of whether the person was in hospice care. Special bereavement services for grieving children.
  • Music Therapists, who provide comfort, meaningful connections with loved ones and symptom relief through music.
  • A legion of volunteers, to provide additional support to you, your family and caregivers
  • Care provided wherever you are, whether a private home, an assisted living facility, a nursing home, or a hospital
  • Complementary therapies (such as physical, speech or occupational therapy, and dietary counseling) individualized to meet your unique needs
